I will not share your personal information with anyone else unless there is a duty to disclose in order to comply with any legal obligation (terrorism alerts, money laundering).
Or cases of emergencies where disclosure might be necessary, for example, risk of harm to yourself or third party. In this instance, I may need to contact your GP or next of kin, with your knowledge and consent.
(UNLESS it is a case of terrorism or money laundering, which by law I must report without the need for consent).
Supervision
Within my monthly supervision sessions, Initials or aliases are used only. My Supervisor is held by the same privacy, data protection, and confidentiality clauses as described here, and in my contract.
How the information and data is stored
Digital Information such as names, email addresses, and telephone numbers are stored via password-protected electronic systems.
Written data such as contracts and process notes are stored via a locked storage cabinet, in which only I have the available key.
Any stored information is kept for 7 years after the service relationship has ended, as required by my professional insurance provider.

Data regarding enquires is deleted immediately after correspondence has ended, and services have not been required.

Right to Access
You have the right to ask for a copy of any personal information attained by me, free of charge, in
an electronic or paper format. This information will be provided by me, within one month of your request. You also have the right to ask for amendments or changes to be made to any incorrect information about you.
Right to be Forgotten
You have the right to ask any information that I hold about you to be erased. This includes your personal information that is no longer required for the services, or if you wish to withdraw your consent from the retainment of your data.. These rights are obligatory unless the information held has a legal obligation to be retained.
Data Portability
You have the right to receive your personal information which you previously provided, and also have the right to transfer that information to another party. For the purposes of the General Data Protection Regulations (GDPR) 2018, the data “controller” is Emma Daniels, Belper Counselling, and Wellbeing.
